Bring a large pot of well-salted water to a boil.
Cook tortellini according to package instructions until al dente.
Reserve ½ cup pasta water, then drain.
In a wide pan over low to medium heat, melt butter.
Add garlic and sauté gently for 30 seconds until fragrant (do not brown).
Pour in cream and warm gently for 3–4 minutes.
Lower heat and stir in Parmesan a little at a time, stirring constantly until melted and smooth.
Season with black pepper and nutmeg if using.
Add tortellini to the sauce and gently toss to coat.
Add a splash of reserved pasta water if needed to loosen the sauce.
Heat for 1–2 minutes until perfectly coated.
Divide tortellini between warm plates.
Finish with extra Parmesan, herbs, and cracked black pepper.
Serve immediately.
